The first step to knowing what treatment you need is determining the cause of your ulcer and whether it’s caused by a venous or arterial disease, since each will have different treatments. Your doctor will have you undergo certain tests, such as the Doppler test--which analyzes the blood flow in your legs--to see what may be causing the ulcer. During a Doppler test, a doctor compares the blood pressure in your ankles to the blood pressure in your arms. This test is used to confirm it’s caused by venous disease or another issue rather than arterial disease.
To confirm it is caused by a venous disease, we will examine your legs for varicose veins while you are standing up, since they are more visible, and then they will have you lie down to examine the ulcer, which becomes more visible when you are lying down. They will also ask you about your health history to determine if any other of your previous health issues are causing it. From here, we’ll determine what your treatment plan should be.
ELT is performed for the treatment of large, bulging varicose veins and their underlying cause, the venous valve reflux. In the last few years, the use of Laser has become an accepted alternative for surgical stripping. With ELT, no hospital admission or surgery is required and the entire procedure can be performed in less than 20 minutes. Briefly described, is done under local anesthesia by using Ultrasound guidance to thread a small Laser fiber into the diseased vein. The physician then delivers laser energy through the fiber which causes the vein the close as the fiber is gradually removed. After the procedure, a pressure stocking is worn for 24 hours. Thereafter, a compression hose is worn for approximately 7-14 days. Immediately after the procedure, the patient encouraged to walk and resume usual activities (patients however should avoid swimming, hot baths or vigorous gym workouts for the following two weeks only.) The affected veins disappear over a period of one month.
